company As a RN Case Manager You will: - Assess/monitor physical,
emotional, and psychological needs of patients - Create hospice
care plans that align with the patients wishes and goals - Direct
nursing care: administering medications, treatments, and
interventions - Provide pain and symptom management - Educate and
support the patient's family and caregivers - Collaborate with
interdisciplinary team - Maintain accurate and timely documentation
- Participate in on-call rotation as required by local branch

employment status Qualifications - Graduate of an accredited
nursing school with current licensure to practice in state of
operation - One year nursing experience required, 3 years preferred
- Experience in hospice or a similar setting preferred - Knowledge
of the hospice philosophy of care - Commitment to clinical and
documentation excellence - Must be a licensed driver with an
automobile that is insured in accordance with state/or organization
