Position Details - Setting: Home Health - Location: Meyersdale, PA and surrounding communities - Start Date: ASAP - Schedule: Flexible – full-time or part-time options - Mileage: Reimbursed - Caseload: Adult and geriatric population Primary Coverage AreaYou will serve patients within the Meyersdale region, including but not limited to: Primary ZIP Coverage: 15411, 15530, 15532, 15424, 15538, 15540, 15542, 15551, 15552, 15557, 15558, 15562, 15565, 15424, 15545/15564, 15437, 15459, 15401 Communities Served: Addison, Berlin, Boynton, Brothersvalley, Callimont, Casselman, Confluence, Deal, Fairhope/Kegg, Fort Hill, Garrett, Glencoe, Hamedsville, Kingwood, Listonburg, Macdonaldton, Mance, Markelton, Meyersdale, New Lexington, Pleasant Union, Pocahontas, Rockwood, Roxbury, Salisbury, Sand Patch, Savage, Sclutton, Springs, Summit Mills, Trent, Ursina, Wellersburg, West Salisbury, Farmington, Markleysburg, Uniontown, New Centerville. (Agency coverage allowed within 1 hour of the Meyersdale office and when wounds are not complex or requiring IV support.) Responsibilities - Evaluate, treat, and manage home-based physical therapy patients - Develop individualized care plans - Collaborate with the clinical team and communicate progress - Document timely and accurately in accordance with home health standards - Provide patient and caregiver education to support safe, effective care Qualifications - Active Pennsylvania Physical Therapist license - Minimum 1 year of home health experience preferred - Strong clinical judgement and ability to work independently - Reliable transportation and valid driver’s license Would you like guaranteed pay for an entire year?
