the Role During visits that may last up to one hour, our providers
assess a member’s current health status, medical history,
medication adherence, social environment, and other risks. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Comprehensive Health Assessment (CHA) enhances care quality and
allows us to address multiple care gaps in a single visit. Our
providers will: Perform assessments for adults and geriatrics,
including medical history, diagnosis, treatment, health education,
physician referrals, case management referrals, follow-ups, and
thorough documentation in line with our protocols. Administer and
order necessary point-of-care and lab testing. Collaborate with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as needed. Work with
Primary Care Physicians (PCPs) on patient education and follow up
as required. Provide services at the member’s residence, which may
include their homes or Skilled Nursing Facilities. Benefits of

Schedule visits according to YOUR availability. Assessments are
conducted from Sunday to Saturday, 8 AM to 7 PM, allowing you to
work during the day, evening, or weekend as it suits you.
Competitive Pay – Your earnings are based on the number of
assessments completed. Increase your income by being available for
more visits and traveling to see members outside your home area.
Those averaging 20 to 100 visits monthly can earn between $1,800
and $11,500 , depending on the per-assessment rate of $90 to $115
based on your state. Benefits – Includes mileage reimbursement,
401(k), employee referral bonuses, and limited voluntary benefits.
Education and Support – We invest in your growth! Along with
comprehensive onboarding training, you will be assigned a
preceptor, receive personalized feedback, and have free access to
continuing education opportunities. Additional Licensure – If
business needs arise, we will assist you in obtaining additional
state licensure and credentialing in neighboring states or others
you may wish to visit while conducting health assessments.
Full-time Opportunities – PRN providers have the option to
transition to full-time roles supporting their home location or a
specific state or region, along with a full-time sign-on bonus.
Qualifications Essential Skills & Experience Master’s Degree or
equivalent experience with successful completion of NP licensure.
Current NP licensure in the state of practice, including
prescription authority or the ability to obtain it as needed. AANP,
AACN, or ANCC board certification in Adult, Adult Gerontology
Primary Care, Adult Gerontology Acute Care, Family, Emergency, or
Acute Care specialty. Current BLS, ACLS, or CPR certification.
Preferred one year of NP experience; new graduates are encouraged
to apply! Valid state driver’s license and proof of adequate
automobile insurance for the state of residence. Strong computer
skills and familiarity with health/medical record software.
