Job Description:

inpatient rehabilitation hospitals and home health agencies, our
Case Managers play a critical role in facilitating patient care
from admission to discharge. The ideal candidate will ensure smooth
transitions for patients receiving home-based or facility-based
care, optimize patient outcomes, provide emotional support, and
work collaboratively with medical professionals and families.
Assess patient needs and develop individualized care plans
Coordinate with healthcare providers to ensure the implementation
of effective treatment plans Manage patient care logistics from
admission to discharge Advocate for patients' needs and maintain
continuous communication with their families Monitor patient
progress and adjust care plans as needed Document patient status
and care provided Ensure compliance with healthcare regulations and
standards Strong interpersonal and communication skills Ability to
coordinate and prioritize effectively Strong problem-solving skills
Comprehensive knowledge of healthcare regulations and policies
Proficient in medical software and patient management systems
Medical Insurance Dental Insurance Vision Insurance 401k Certified
Case Manager (CCM) preferred Valid nursing or social work license
in the state of practice
