We are currently seeking a committed Case Manager to join our dynamic team in the Medical Services department. The ideal candidate will manage patient care coordination, ensuring seamless patient experiences across our rehabilitation hospitals, home health agencies, and hospice services. This role involves assessing patient needs, collaborating with healthcare professionals, and providing patient and family education. Assess patient needs and develop personalized care plans Coordinate with physicians, nurses, and other healthcare professionals to implement effective treatment plans Monitor patient progress and adjust care plans as necessary Provide patients and families with education about treatment options and healthcare management Ensure compliance with healthcare regulations and maintain patient confidentiality Document patient care activities and outcomes for clinical and billing purposes Strong interpersonal and communication skills Proficiency in electronic health record (EHR) systems Ability to work collaboratively in multidisciplinary teams Strong organizational skills and ability to handle multiple cases Knowledge of healthcare regulations and patient rights Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) preferred Licensed Registered Nurse (RN) in applicable state, if nursing background
