Center is hiring for a Nurse Care Manager to provide a variety of
patientcare activities and supportive services in collaboration
with the medical staff and care teams. This position is full time
but candidates seeking less than full time employment are also
encouraged to apply. About NSCHC: North Side Christian Health
Center is a comprehensive primary healthcare center established in
1993. NSCHC serves all patients without regard for their ability to
pay. We serve everyone seeking the best care available regardless
of their religious beliefs, ethnic and cultural background,
language spoken, age, sex, and disability status. Our staff
provides primary and preventive medical care for patients of all
ages, mental and behavioral health services, dental care, and case
management. DUTIES/RESPONSIBILITIES - Assist patients with the
management of their care (this is not a management position).
Demonstrates appropriate judgment skills to be able to make
independent clinical decisions in routine patient care matters
Ensures the safe, productive, and efficient operation of the clinic
through timely and attentive administrative and clinical support
Demonstrates skills in phlebotomy, obtaining EKG's, injectable
medications including immunizations, and administering IM
antibiotics. Performs case management functions; coordinating with
internal and external resources to support patient Demonstrates
prioritization of patient needs and patient flow, phone triage,
provider support, nursing assessment, patient education, patient
scheduling, and referrals Triages patient situations and
coordinates care with physician. Assist physician with all aspects
of patient care including assessment, evaluation and education.
Demonstrates critical thinking in the identification of clinical,
social, safety, psychological, spiritual and environmental needs
for the patient care within an episode of care. Practices solid
communication skills, and is able to articulate and translate the
patient's condition to other care providers and to negotiate and
make recommendations for changes in patient care and office
practices. Patient documentation is comprehensive and promotes
communication between caregivers. Incorporates national
professional organization as well as business unit and health
system's goals to improve patient safety, quality and satisfaction.
Fosters an environment for open communication and collaborative
practice, communicates effectively in oral and written formats
Demonstrates accountability for professional development that
improves the quality of professional practice and the quality of
patient care. Actively participates in practice-based shared
governance, goal setting and supports the change and transition
process to improve quality of care and the practice environment.
Participates in work that improves patient care and the
professional practice environment. Actively participates in
department or practice-specific quality improvement efforts. Takes
personal responsibility in improving patient satisfaction with the
quality of care and service. Utilizes research and evidence-based
practice to support improvement in clinical care. Demonstrates
knowledge of adult learning principles and applies in teaching of
patients, families, students and new staff. Provides detailed and
appropriate teaching to patients and families to effectively guide
them through the episode of care both in the office setting and via
phone/written communication when at home. Supports the development
of students, new staff and colleagues. Perform other duties, as
assigned by manager. Occasional local travel between clinic sites.
KNOWLEDGE/SKILLS/ABILITIES - Ability to take initiative appropriate

cooperatively as a team member Develops and maintains productive
working relationships internally and externally by demonstrating
accountability for actions, enthusiasm, motivation and commitment
to patients and colleagues. Demonstrates an understanding of

relationships. Demonstrated success in customer service/patient,
preferably in a medical care facility Demonstrated success in
managing difficult customer situations Strong computer skills
including Outlook, Word, Excel and PowerPoint preferred.
Significant knowledge of medical practices and procedures in a
primary care environment Commitment to NSCHC faith-based mission to
provide care to all who request it, without judgment.
QUALIFICATIONS (REQUIRED FOR EMPLOYMENT) - Current Pennsylvania RN
licensure required Current CPR certification child and adult, BLS
required Experience in outpatient and/or community setting
preferred Experience with pediatrics and adults highly desirable
Act 153 Clearances Annual influenza vaccine Must obtain or provide
proof of (within 14 days of hire): PPD, Tdap, Hepatitis B Immunity,
MMR Immunity, Varicella Immunity
