Qualifications What You Will Do As an RN Case Manager, you will play a vital, hands-on role in providing compassionate hospice care, including: Performing physical and psychosocial assessments Identifying hospice diagnoses and documenting ongoing eligibility Delivering skilled nursing care Assessing and managing pain and symptoms Ensuring the highest level of comfort and quality of life Educating patients and families on end-of-life processes and rights Developing and coordinating individualized plans of care Initiating referrals and providing ongoing emotional and palliative support Supervising hospice aides per Medicare requirements Previous hospice experience strongly preferred. What We’re Looking For Registered Nurse (RN) licensed in Pennsylvania, in good standing 1 year of recent hospice experience preferred Strong communication, customer service, and critical thinking skills Ability and willingness to travel within the service area Valid PA driver’s license, personal vehicle, and insurance Ability to lift/move 50 lbs and transfer patient weight of 100 lbs Ability to perform physical job functions (walking, bending, standing, etc.) Reasonable accommodations may be made to enable individuals with disabilities to perform essential job functions without compromising patient care.
