The successful candidate will support rotating shifts and days, on-call expected. Purpose: Provides anesthesia services to all areas of the hospital in accordance with standards set forth by the Department's Policies and Procedures and approved by the Medical Board of the hospital. Each CRNA is accountable to function under the supervision of an Anesthesiologist. Responsibilities: ---Assumes responsibility for own professional development through active participation in inservices and /or continuing education programs. a. Attends inservice programs, outside conferences, courses and programs. b. Attends patient centered conferences. c. Shows evidence of independent study by reviewing current nursing literature and sharing information at a minimum of one unit-bases meeting a year. d. Obtains a minimum of 40 CEU per 2 years (IU = 1hr.) e. Shares information obtained through educational programs at unit meetings and/or scheduled inservices with peers. ---Complies with the organization's established policies. a. Complies with established Human Resources policies. b. Complies with established Nursing Service policies. c. Complies with the established chain of command for work-related problems and/or suggestions. d. Prevents and/or minimizes risk from disasters, fires, environmental hazards, electrical hazards, and infection. e. Utilizes unit communication system such as staff meeting, memos, bulletins, minutes, and/or communication books to obtain current information. ---Demonstrates knowledge of the problem solving process in the identification and resolution of patient care problems a. Anticipates and identifies problems and concerns. b. Generates effective solutions. c. Refers problems when they are beyond their scope of authority or ability utilizing the accepted chain of command. ---Demonstrates skills and knowledge necessary to assure assessment, treatment and care is appropriate to the age of the patients served. a. Demonstrates the ability to obtain and interpret information in terms of the patient's needs. b. Demonstrates knowledge of growth and development, and the ability to appropriately utilize that knowledge in providing care for individual patients and identify the special anesthetic needs of children and elderly. c. Demonstrates an understanding of the range of treatment needed and the ability to provide that care for individual patients. ---Documents anesthesia care in accordance with department/hospital policies a. Documents the preanesthetic evaluation and informed consent for anesthesia. b. Documents, as part of the medical record, all aspects of anesthesia related care in a thorough and timely fashion. c. Documents intra-operative consultation with anesthesiologist and surgeon. d. Documents any relief anesthetic care provided and name of care giver. ---Essential Functions continued: e. Provides appropriate perianesthetic invasive and non-invasive monitoring utilizing current standards and techniques and responding to abnormal findings with corrective action. f. Recognizes and treats cardiac dysrhythmias through the use of perianesthetic electrocardiogram monitoring. g. Recognizes abnormal patient response during anesthesia, selecting and implementing corrective action and requesting consultation whenever necessary. h. Manages the patient's fluid, blood electrolyte and acid-base balance. i. Evaluates patient response during emergence from anesthesia and institutes pharmacological or supportive treatment to insure the adequacy of patient recovery from anesthesia and adjuvant drugs. j. Provides anesthesia care consistent with infection control and anesthetic safety principles in order to prevent the spread of disease and prevent harm to the patient, the anesthetist and other in the anesthetizing environment. ---Essential functions: Demonstrates skill to administer safe and smooth anesthetic care to all patients, in all areas of the hospital where Anesthesia coverage is provided. a. Obtains, prepares, or utilizes all equipment, monitors, supplies and drugs used for the administration of anesthesia; performs or orders safety checks as needed. b. Selects, obtains or administers the anesthetics, adjuvant drugs, accessory drugs, and fluids necessary to manage the anesthetic, to maintain the patient's physiologic homeostasis, and to correct abnormal responses to the anesthesia or surgery. c. Demonstrates expertise in Airway Management, including mask ventilation, tracheal intubation, laryngeal mask airway placement, and maintenance of appropriate mechanical ventilation. d. Manages regional anesthetic techniques including, but not limited to, subarachnoid, epidural and caudal blocks; plexus major and peripheral nerve blocks; performs and manages intravenous regional anesthesia. ---Participates in Department and Safety Program a. Performs all tasks safely and without endangering self or other. b. Assumes responsibility and accountability for safety of self and safety of others (employees, patients, visitors) as applicable. c. Employs acceptable safe work practices in completing all tasks duties and responsibilities. d. Attends OSHA inservices. ---Performs nursing activities utilizing equipment and supplies according to hospital policies. a.Knows the location, operation, availability and care of equipment necessary to function in area of assignment. b.Problem solves, trouble shoots, and takes corrective measures in care and handling equipment. c.Participates in evaluation process of equipment/supplies. d.Recognizes the need for cost effectiveness without jeopardizing patient care. ---Performs required nursing procedures consistent with scientific principles and hospital policies in area of assignment. a. Demonstrates competency in performance of required nursing procedures. b. Adapts procedures to meet individual patient needs. c. Identifies own capabilities and limitations and utilizes appropriate resource to assist with effective intervention when encountering a new or unfamiliar situation/equipment. ---Provides anesthesia support to all areas of hospital a. Responds to emergency situations by providing airway management, administration of emergency fluids or drugs, or using basic or advanced cardiac life support techniques. b. Works within scheduling pattern to assure adequate anesthesia support based on department staffing guidelines c. Supports established on-call system, assuring availability to institution's needs within 20 minutes. d. Utilizes on-call staff to meet OR and LED needs. e. Demonstrates flexibility in daily assignment alterations, based on anesthesia needs throughout the institution. ---Satisfactorily manages anesthesia process to include induction, maintenance and emergence. a. Identifies the patient and introduces self as a nurse anesthetist. Discusses the proposed surgery and anesthesia with attending Anesthesiologist. Ascertains that permits, lab studies and tests are present on chart. Notify MD in case of inconsistencies of data reviewed. b. Has resuscitative equipment readily available for every patient receiving any type of anesthetic care. c. Selects, applies or inserts appropriate non-invasive and invasive monitoring modalities for collecting and interpreting patient physiological data. d. Adequately manages a patient's airway and pulmonary status using mask ventilation or endotracheal intubation, mechanical ventilation, pharmacological support, respiratory therapy, or extubation. e. Identifies own capabilities and limitations and recognizes clinical situations which require MD intervention. f. Manages emergence and recovery from anesthesia by selecting and administering medications, fluids or ventilator support in order to maintain homeostasis, to provide relief from pain and anesthesia side effects, or to prevent or manage complications. ---Satisfactorily manages anesthesia process to include induction, maintenance and emergence. g. Inserts arterial catheters and performs arterial puncture to obtain arterial blood samples. h. Identifies and manages emergency situations, including initiating, participating in cardiopulmonary resuscitation that involves airway maintenance, ventilation, tracheal intubation, pharmacological cardiovascular support, and management of blood, fluid, electrolyte and acid-base balance and insertion of invasive monitors as needed. i. Provides a thorough report of the patient's psychological and physical condition, perianesthetic course and anticipated problems to the postanesthetic health care provider who assumes the patient's care following anesthesia. j. Provides post anesthesia follow-up and evaluation of the patient's response to anesthesia and for potential anesthetic complications taking appropriate corrective actions and requesting consultation whenever indicated. k. Initiates and administers respiratory support to insure adequate ventilation and oxygenation in the immediate post anesthesia period.Qualifications Education and Training: Graduate of approved School of Nursing. Graduate of approved Program of Nurse Anesthesia. License Certification & Registration: Current Pennsylvania Registered Nurse License. Current NBCRNA Certification. Experience: Anesthesia experience preferred Graduate Nurse Anesthetists: Must receive certification within 3 months of graduation from an approved program of Nurse Anesthesia.
