Job Description:

The purpose of this position is to serve as a Licensed Practical Nurse LPN - Patient Aligned Care Team Float ( (PACT) assigned to the Outpatient Service performs age-specific nursing care for adult and geriatric patients with physical, mental, behavioral, or psychosocial needs, according to assignment and training. The LPN implements nursing orders, as per established policy and procedure which have been developed by the Registered Nurse (RN) in providing nursing care to these patients. [ "This LPN position is required to float between all 3 Altoona Primary Care Teams (Blue, Red and Yellow), as well as all 5 Community Based Outpatient Clinics (CBOCs) located in DuBois, Huntingdon, Indiana, Johnstown & State College, PA. The duty station will be Altoona, PA. The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic life care plan. The LPN demonstrates active collaboration and communication with Core Team (patient, Primary Care Provider, RN Care Manager and Clerk) and Expanded Team (family/ caregiver, internal and community-based services involved in providing care to the patients).\nDuties include but are not limited to:\na. Technical proficiency in initiating, performing, and completing assigned duties in providing care to variable patient populations.\nb. Knowledge and ability to appropriately carry out assigned patient care based on the patients conditions; to use sound judgment in selecting the appropriate sequence and order of procedures and treatments; and to accurately recognize, report, and record relevant patient information.\nc. Ability to observe, identify, and respond to the patient's needs for care including medications and equipment assisted care and patient/family education.\nd. Recognizes and considers emotional, cultural, spiritual, socioeconomic, and age-related factors in care delivery.\ne. Prepares and administers prescribed medications and performs treatments according to established policies/procedures. Observes for physical and/or emotional changes in the patient'sFUNCTIONAL STATEMENT OUTPATIENT SERVICE SCRIBE LICENSED PRACTICAL NURSE (GS-6) conditions from prescribed medications/treatments. Promptly and accurately documents noted changes and reports any deviations from normal to the RN or MD/DO.\nf. Knowledge and ability to recognize urgent or emergent situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.\ng. Knowledge and understanding of human behavior, patient motivations, and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff.\nh. Established constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication, and socialization skills.\ni. Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery.\nj. Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or internal/external customers.\nk. Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized procedures.\nI. Responsible for delivering patient care in a variety of settings.\nm. Utilizes a Veteran-Centered Care approach to gain an understanding of the Veterans needs and desires.\nn. Member of the core team and may collaborate with services internal and external to the VA to facilitate care transition in order to effectively meet the patient's needs. The LPN performs health education and basic health coaching to promote patient-driven goal setting and patient activation for self-care and self-management using TEACH and Motivational Interviewing.\no. He/she advocates for patients, is a team member who promotes his/her own professional growth, development and practice. The LPN maintains flexibility in order to manage additional duties as assigned. p. Responsible for demonstrating the knowledge and practice of providing Nursing Care based on the four performance standards/ competencies, nursing care; safety; interpersonal communication; and customer service.\nq. Maintains confidentiality of electronic, written, and /or verbal information.\nr. Performs, reports, and records assigned nursing care including, but not limited to policy orders, orders in suspense, protocols, completion of clinical reminders, appropriate consults and alerts as indicated.\ns. Observes and provides care for patients through the implementation of assigned interventions. These interventions are targeted toward maximizing self-care management abilities. The LPN will always arrive to Altoona (or if your residence is closer to the desired location for the day you may leave from your residence) then drive on the clock to the coverage location or when needed arrive at the coverage location for the start of your tour and end of the tour. Work Schedule: Tour of duty is varying daylight hours, Monday through Friday; however, weekend and evening hours may occur when Veteran needs arise. Travel to Altoona VAMC and other CBOCs as needed if staffing needs arise but at the time of travel the individual will be reimbursed according to local guidelines.
