    • BLS required for all travelers here.
    • Check UD to determine if ACLS, PALS required for other applicable units.
    • Primary source online verification required.
    • Physical within 1 year of start - uploaders use 3/3/3000 expiration date.
    • Candidate to complete quiz before start.
    • Candidate must review and sign orientation packet before start.
    • Medical Surveillance Checklist must be completed.
    • Complete form if declining COVID vaccines.
    • Complete form if declining MMR/V/HepB/TDAP.
    • Negative 10 Panel Drug Screen within 30 days of start.
    • Vaccine or facility specific declination required during flu season.
    • COVID-19 vaccine documentation required.
    • Alcohol breathalyzer test within 30 days of start.
    • 7 Year Criminal Search required within 1 year of start.
    • Fingerprinting required - candidates must complete in PA.
